We make every effort to give the best service possible to everyone who attends our practice.

However, we are aware that sometimes there may be cause for concern or ideas about how we could better run things. If any of these apply, we would wish for the matter to be dealt with as soon as possible.

You can complete this questionnaire more than once in your time with the practice but we would ask that you not submit the same feedback more than once in any year.

Making a suggestion

To make a comment or suggestion as to how we can improve our service to you, please click on the Suggestions Form button and complete the form to send to us.

Medical matters and official complaints cannot be dealt with via this form. If you have a query regarding a medical matter please telephone reception to make an appointment to see the appropriate person.

You can also contact our Patient Participation Group (PPG) with whom we consult regarding changes/improvement to services.

If you would like to make an official complaint to us, please see our Complaints Leaflet which explains the process to follow.  We believe this will give us the best chance of putting right whatever has gone wrong and provide an opportunity to improve our practice.
