New Patient Registration

Our practice area covers St. George, Speedwell, Kingswood, Soundwell, Hanham, Crew’s Hole and parts of Fishponds, Mangotsfield, Redfield and Whitehall.

To register as a patient with Fireclay Health, you must live within our practice boundary - enter your postcode in our map tool below to check your eligibility.  Smartphone users are recommended to use this link to open map tool in a new window.  You can move map in any direction and zoom in/out as required.

We require both a Registration form and a Medical Questionnaire to be completed in order to register you.  Please add your NHS number to registration form if known - this ensures a correct match to the NHS central patient registry and therefore that we obtain any previous medical records for you.  You do not have to show us ID documents however assurances of your identity and where you live may be needed for correct matching.

There are two different options for completion:

Option 1 - Online forms (generic):

Use the links below to complete both forms, clicking the 'send' button each time to send the form electronically to us. Note that this involves transmitting information about yourself across the Internet - although every effort is made to keep this information secure, no guarantee of confidentiality can be offered.

Use the links to view and either print for completion and bring with you to the practice, or request forms from reception to complete when you visit.

On receipt the Admin team will process your registration - please allow two working days for this, then you will be able to make an appointment with us if needed.  

Please register new babies with us as soon as possible after birth - complete online or print versions of the registration form and Child questionnaire (including first language child will be brought up with)

Organ & Blood Donation

Although the back of the GMS1 form includes options to register to be an organ and/or blood donor, this can no longer be acted on locally. We ask that you register yourself if interested - telephone NHS Blood and Transplant on 0300 123 23 23 or register online:

To register for organ donation, amend registration, refuse to donate or withdraw consent. From spring 2020, all adults in England will be considered to have agreed to be an organ donor when they die unless in one of the excluded groups or opted out

New blood donors must be aged between 17- 65. Registering online also gives you the ability to book and cancel appointments online, and view your recent donation history.
