NHS Health Checks

The NHS Health Check is your chance to get a free midlife MOT. It's designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia. As we get older, we have a higher risk of developing one of these conditions. An NHS Health Check helps find ways to lower this risk.

Eligibility criteria;

  • aged 40-74 without a pre-existing condition*
  • aged 30-39 at higher risk of developing a circulatory/vascular condition (a local extension of the Health Check programme. A small number of patients have been identified as eligible for these reasons and will be specifically invited - please do not book unless you have been invited to do so)

*Patients known to have cardiovascular disease are reviewed separately and are not eligible for this check.  This includes patients diagnosed with hypertension (high blood pressure) and patients taking statins.

Health Check leaflet

Health Check results booklet - you will be given a completed version of this at the end of your check

The check involves straightforward questions and measurements such as age, sex, family history, height, weight and blood pressure. There will also be a simple blood test to measure your cholesterol level.  Following the check, you will receive free personalised advice about what you can do to stay healthy.

It will save time in your appointment if you could download the below questionnaires to complete in advance and bring with you:

AUDIT (1 page pdf) Alcohol Use questionnaire

GPPAQ (1 page pdf) General Practice Physical Activity Questionnaire - to calculate your activity level, see the NHS Prescription for Health website

We send out postal, email and/or text message invitations but it will necessarily take a while to invite all our eligible patients.  If you are in the target age range but have not received an invitation letter please telephone and a Receptionist will check if you are eligible before booking an appointment if appropriate.

If you do not want a health check, we would be grateful if you could let us know to save us contacting you.  Please note that once you have had a Health Check you will not be eligible to receive a Health Check again for five years.
