New Patient Information
Please read our Practice Leaflet (2 page pdf)
All patients, including children, are allocated a named, accountable GP, referred to as your 'Usual GP' - you should be advised who this is when you register. We encourage you to see your Usual GP where possible, particularly for on-going problems, so we can provide you with better continuity of care. Please note, we do not ask new patients to make a first appointment for a general health check. If your Usual GP is fully booked, you can ask for an appointment with another Doctor or a Minor Illness Practitioner. We will make reasonable efforts to accommodate requests to change Usual GP.
This practice is signed up to the Electronic Prescription Service (EPS) - if you have previously nominated a pharmacy which is no longer convenient for you to attend, please ensure you update your nomination by telling us or your new pharmacy.
Once you are registered with the Practice
Please subscribe to our Patient Newsletters and consider becoming a member of our Patient Participation Group - you can easily sign up for this online and be as much or as little involved as you wish
Your details will be added to our computer system and your patient record will be kept electronically on our practice systems. Please ask at Reception if you would like to register for Online Services which will enable you to book appointments and request repeat medication online - if you set this up with your previous GP practice it will not transfer, though you are likely to be able to add our practice to your account in order to use the same log-in details once you have received your registration information from us.
We advise you read our Confidentiality & Data Sharing information - this includes details about how to opt out of sharing your data if you have concerns on this matter.
Additional Practice Information
GP practices are part of the National Health Service (NHS) - there are some useful resources explaining how the NHS works on the Kings Fund website. The history of the Fireclay Health GP Practice is available via our Further Information pages.
We are a research-active organisation - currently Level 2 in the National Institute for Health Research's (NIHR) Research Site Initiative scheme (as part of the West of England Clinical Research Network). Read more about taking part in research here and/or in the NIHR's OK to Ask leaflet (pdf, easy-read format). For more details about clinical trials, see the UK Clinical Trials Gateway website - if you find a trial you’re interested in and which is currently recruiting people to take part, you can contact the trial coordinator to register your interest.
All GP practices are now required to declare the mean earnings for GPs delivering NHS services to patients in the practice for longer than six months in the previous financial year - this information can be found on our GP Earnings Further Information page
